We are thrilled to welcome you as a new patient to the Children's Clinic. It is our goal to provide you with excellent medical care with compassion, professionalism, and confidentiality to the best of our ability.

We also invite parents and prospective patients to schedule a complimentary 'greet and meet' appointment with one or more pediatricians. This is an informal visit where you can get a feel for the clinic as well as the individual physician's personality and medical care philosophy. Feel free to prepare questions in advance.

In order to make your experience a positive one, the following are some policies and guidelines that may help us meet your needs quickly and smoothly.

  • If it is the first visit for your child, we request that you arrive a little early to allow for time to fill out patient information. Please bring your insurance information, your child's immunization card and, if possible, his/her medical records. If you do not already have a copy of your child's medical records, you will be able to sign a medical records release form so that we can receive your child's medical records from his/her previous physician. Also, be sure that a parent or legal guardian accompanies the child. It is important that we meet the primary caregivers and gather as much medical history as possible to give your child the best care.
  • If a parent or legal guardian is unable to accompany the child for his/her visit, please be sure that we receive a signed authorization to seek medical care for your child. This allows your designee to bring your child in and act on your behalf to consent to medical procedures and treatments for your child. We are unable to examine and treat your child without your written consent.
  • Please bring your medical insurance card with you for each doctor's visit. Verification of insurance eligibility will ensure a smooth process for billing or possible referrals.
  • Please inform us of any changes in your insurance, address or telephone numbers.
  • If your insurance requires a co-pay, we must honor that. You will be requested to pay your co-pay at the time of service. For your convenience, we do accept cash, checks, and credit cards.
